Laboratory Testing Before Alteplase Administration in Acute Myocardial Infarction

Essential Pre-Treatment Laboratory Testing

  • The American Heart Association recommends that only blood glucose measurement is mandatory before initiating alteplase in all patients, as this is the single test that must be obtained before treatment 1, 2.

When Coagulation Studies Are Required

  • Coagulation studies, including CBC, PT/INR, and aPTT, should be waited for before administering alteplase only in specific circumstances, such as clinical suspicion of bleeding abnormality or thrombocytopenia, known warfarin use, known heparin use, or uncertain anticoagulation history 3, 4.
  • The American College of Cardiology recommends that PT/INR must be ≤1.7 and PT <15 seconds by local laboratory standards in patients with known warfarin use 5.

Contraindications Based on Laboratory Values

  • Alteplase should not be administered if coagulation studies reveal platelets <100,000/mm³, INR >1.7, aPTT >40 seconds, or PT >15 seconds 5, 6.
  • The American Heart Association recommends sending a complete blood count with platelets, serum electrolytes and renal function, cardiac biomarkers, and coagulation studies, but these should not delay treatment 3, 4, 7.

Critical Timing Considerations

  • Time is muscle in myocardial infarction, and every minute of delay increases mortality, with the benefit of alteplase greatest when administered early, up to 12 hours after symptom onset 8.
  • Treatment can be initiated before laboratory results are available, but should be discontinued if abnormal values return, according to the American College of Cardiology 5, 3, 4.

Common Pitfalls to Avoid

  • The European Society of Cardiology recommends never delaying treatment for routine laboratory testing in patients without bleeding concerns or known anticoagulant use 3, 4, 2.
  • DOAC use within 48 hours is a contraindication unless specific factor Xa or thrombin assays are normal and renal function is normal, as stated by the American Heart Association 5, 6.
