Diagnosis of Stage F2 Fibrosis

Diagnostic Methods for F2 Fibrosis

  • The American Association for the Study of Liver Diseases recommends using a combination of liver elastography and blood-based biomarkers for the diagnosis of stage F2 fibrosis, as liver biopsy is invasive and has limitations in diagnosing intermediate stages of fibrosis 1, 2
  • Transient Elastography (FibroScan) is the most validated imaging technique for F2 fibrosis, with a sensitivity of 75.1% and specificity of 79.3% for significant fibrosis (F2-F4) with a cutoff >6.0-8.0 kPa 2, 3
  • The European Association for the Study of the Liver suggests that Shear Wave Elastography (SWE) is a good alternative to Transient Elastography, with a sensitivity of 85-93% and specificity of 86-91% for HCV patients with F2-F4 fibrosis 2
  • Magnetic Resonance Elastography (MRE) is considered an accurate non-invasive imaging technique, with cutoff values for F2 fibrosis ranging from 3.3-4.2 kPa 4

Blood-Based Biomarkers

  • The APRI (AST-to-Platelet Ratio Index) has a low cutoff (>0.3-0.7) with a sensitivity of 72.9% and specificity of 64.7% for F2-F4 fibrosis, and a high cutoff (>1.3-1.7) with a sensitivity of 30.5% and specificity of 92.3% for F2-F4 fibrosis 3
  • The FIB-4 Index is better than APRI for identifying F2-F4 fibrosis, with a FIB-4 <1.45 being better than APRI <0.5 for ruling out F2-F4 fibrosis 1
  • The Enhanced Liver Fibrosis (ELF) Score has a cutoff of 9.8 when used in isolation, and values between 9.2-9.7 require additional non-invasive testing to confirm F2-F3 fibrosis 5
  • The initial assessment should start with blood-based biomarkers (APRI, FIB-4) as screening tools, and if FIB-4 <1.45 or APRI <0.5, there is a low probability of F2+ fibrosis 1, 6
  • If FIB-4 >1.45 or APRI >0.5, proceed to elastography, with Transient Elastography (FibroScan) values between 7-10 kPa suggesting F2 fibrosis 1, 2, 3
  • MR Elastography values between 3.3-4.2 kPa suggest F2 fibrosis, and 2D-SWE values around 7.3 kPa suggest F2 fibrosis 2, 4

Important Considerations and Pitfalls

  • All non-invasive tests have reduced accuracy for intermediate fibrosis stages (F2) compared to advanced fibrosis (F3-F4) or cirrhosis (F4) 1
  • Acute inflammation can falsely elevate elastography readings, and obesity can limit ultrasound-based elastography accuracy 2
  • Using combinations of different non-invasive tests improves diagnostic accuracy, and sequential use of blood tests followed by elastography is recommended 1, 3, 5
  • Different cutoffs may apply depending on underlying liver disease, and for MASH patients, VCTE values of 10-15 kPa suggest F2-F3 fibrosis 1, 4, 5, 6
