Neoadjuvant Chemotherapy Guidelines for Triple-Negative Breast Cancer
Standard Neoadjuvant Approach by Stage
- The National Comprehensive Cancer Network recommends neoadjuvant pembrolizumab combined with carboplatin and paclitaxel, followed by pembrolizumab with anthracycline and cyclophosphamide, then continued as adjuvant pembrolizumab for up to one year regardless of pathologic response for stage II-III TNBC 1, 2
- For stage I TNBC, T1a (≤5 mm) patients may omit chemotherapy, while T1b (6-10 mm) patients may receive taxane-cyclophosphamide regimen, and T1c (11-20 mm) patients may receive anthracycline-taxane chemotherapy with consideration of adding carboplatin and pembrolizumab for higher-risk features 1, 2
Alternative Neoadjuvant Regimens
- The National Comprehensive Cancer Network recommends dose-dense AC followed by paclitaxel every 2 weeks as a preferred alternative regimen 1, 2
- Dose-dense AC followed by weekly paclitaxel and AC followed by docetaxel every 3 weeks are also acceptable sequential anthracycline-taxane regimens 1
Post-Neoadjuvant Therapy Based on Response
- The National Comprehensive Cancer Network recommends adding capecitabine for 6-8 cycles if germline BRCA1/2 wild-type after standard neoadjuvant chemotherapy 1, 2, 3
- Adjuvant olaparib may be added for 1 year in patients with germline BRCA1/2 mutations and ≥pT2 or ≥pN1 disease after adjuvant chemotherapy, or residual disease after neoadjuvant chemotherapy 1
Critical Treatment Principles
- The National Comprehensive Cancer Network recommends obtaining core biopsy confirming invasive TNBC with ER/PR/HER2 status before starting therapy, and referring to breast surgeon and radiation oncologist before initiating neoadjuvant therapy 4
- Testing all TNBC patients for germline BRCA1/2 mutations is recommended to guide adjuvant therapy decisions 3
Common Pitfalls to Avoid
- The European Society for Medical Oncology recommends against using anthracycline-free regimens as standard, and avoiding concurrent anthracycline and trastuzumab 5, 4
- Delaying neoadjuvant therapy beyond 2-4 weeks after diagnosis completion should be avoided 4
- The St. Gallen consensus recommends performing completion axillary dissection if residual macrometastases (>2 mm) are found in sentinel nodes, and axillary dissection for patients with clinically positive nodes after neoadjuvant therapy 6
Neoadjuvant Chemotherapy for Triple-Negative Breast Cancer
Post-Neoadjuvant Therapy Based on Surgical Pathology
- For germline BRCA1/2 wild-type patients with residual disease after surgery, the American Society of Clinical Oncology recommends adding capecitabine 1,250 mg/m² PO twice daily on days 1-14 of 21-day cycles for 6-8 cycles, which improves recurrence-free survival (HR 0.53, P=0.02) and overall survival (HR 0.55, P=0.03) in triple-negative disease with residual disease 7
- Baseline cardiac assessment with LVEF measurement is necessary if anthracyclines are planned, as recommended by the European Society for Medical Oncology 8
Common Pitfalls and How to Avoid Them
- The European Society for Medical Oncology recommends against delaying treatment start, and initiating neoadjuvant therapy within 2-4 weeks of diagnosis completion, to avoid compromising outcomes 8
- Anthracycline-free regimens (carboplatin-taxane) should only be used when anthracyclines are contraindicated (cardiac dysfunction, prior anthracycline exposure approaching cumulative dose limits), as recommended by the European Society for Medical Oncology 8
- Delivering all planned chemotherapy without unnecessary breaks between neoadjuvant and adjuvant phases maximizes the probability of achieving pathologic complete response, which is the strongest prognostic factor, as recommended by the European Society for Medical Oncology 8
