Treatment of Penile Infections in Men Taking Jardiance (Empagliflozin)

Types of Infections and Treatment Approach

  • The Centers for Disease Control and Prevention recommends topical antifungal agents as the first-line treatment for balanitis in men taking SGLT2 inhibitors, with options including clotrimazole cream, miconazole cream, and other azole antifungals 1
  • For more extensive infections, oral fluconazole 150 mg as a single dose may be considered, with a longer duration of therapy needed for recurrent infections 1
  • The Centers for Disease Control and Prevention suggests that for bacterial co-infection or non-responsive cases, treatment for non-gonococcal urethritis may be considered, with options including azithromycin 1 g orally in a single dose or doxycycline 100 mg orally twice daily for 7 days 2, 3, 4

Management and Follow-Up

  • Patients should be advised about proper genital hygiene to reduce the risk of infection 1
  • For recurrent infections, prophylactic antifungal therapy may be considered, and patients should return for evaluation if symptoms persist after completing therapy 1, 3, 4

Warning Signs of Serious Infection

  • Patients should be monitored for signs of spreading infection or Fournier's gangrene, with warning signs including fever 4, severe pain 4, extensive erythema or swelling 1, and systemic symptoms 1
