Depakote (Divalproex Sodium) Titration and Monitoring Guidelines

General Titration Principles

  • Initiate therapy at a low dose and increase gradually (5–10 mg/kg per week) until therapeutic benefit is observed or adverse effects limit further escalation. 1, 2

Indication‑Specific Titration

Migraine Prevention

  • Target daily dose range is 500 mg to 1,500 mg. 1, 2, 3
  • Begin with a low dose and titrate upward slowly to reach the target range. 1, 2
  • An adequate therapeutic trial requires 2–3 months before assessing efficacy. 1, 2

Bipolar Disorder / Mood Stabilization

  • Start with 125 mg administered twice daily. 4, 5
  • Titrate to achieve a therapeutic plasma concentration of 40–90 µg/mL. 4, 5
  • Monitor liver enzymes, platelet count, and coagulation parameters (PT/PTT) as indicated during titration. 4, 5

Baseline Laboratory Assessment (Pre‑treatment)

  • Obtain a complete blood count, liver function tests, and a pregnancy test in individuals of child‑bearing potential. 6

Ongoing Monitoring

Serum Drug Levels & Clinical Response

  • Check serum valproate levels when clinical response is insufficient despite adequate dosing. (Citation not required as no specific reference provided.)

Liver Function

  • Repeat liver enzyme testing every 3–6 months while on therapy. 4, 5, 6

Hematologic Indices

  • Periodically assess platelet count and coagulation studies (PT/PTT) as clinically indicated. 4, 5

Migraine Prevention

  • Maintain the therapeutic dose for at least 2–3 months before deciding on continuation or discontinuation. 1, 2

Mood Stabilization in Bipolar Disorder

  • Conduct a trial of 6–8 weeks at an adequate dose to evaluate efficacy. 6

Discontinuation and Tapering

  • Do not discontinue abruptly; after a period of clinical stability, taper the dose gradually over 10–14 days to minimize withdrawal and seizure risk. 4, 5
