Immediate Treatment for Angioedema in a Child
Assessment and Initial Management
- The American Academy of Allergy, Asthma, and Immunology recommends immediately assessing for life-threatening features, including oropharyngeal or laryngeal involvement, respiratory distress, and hypotension or signs of shock, in children with angioedema 1, 2, 3
Emergency Treatment Algorithm
- For life-threatening angioedema with airway involvement, the American Academy of Pediatrics recommends administering intramuscular epinephrine immediately, with dosing dependent on weight, and securing and maintaining airway patency 1, 2, 3
- The British Society for Allergy and Clinical Immunology suggests administering high-flow oxygen via face mask and giving intravenous hydrocortisone as adjunctive therapy for severe laryngeal edema 3, 4, 5
Treatment for Specific Conditions
- For hereditary angioedema with laryngeal involvement, the European Academy of Allergy and Clinical Immunology recommends C1-INH replacement as the preferred treatment if available, and fresh frozen plasma as an alternative if specific HAE medications are unavailable 1, 2, 6
Supportive Care
- The American Thoracic Society advises avoiding sedatives in patients with respiratory compromise and providing supportive care for specific symptoms, including pain control, antiemetics, and aggressive hydration for abdominal attacks, and symptomatic treatment for extremity attacks 2, 4, 7
Follow-up Care
- The American Academy of Allergy, Asthma, and Immunology recommends observing the child until symptoms have significantly improved and considering referral to an allergist/immunologist for evaluation of the underlying cause 1
