Treatment for Proteus mirabilis UTI

Uncomplicated Cystitis

  • The Infectious Diseases Society of America recommends trimethoprim-sulfamethoxazole (160/800 mg twice daily) as the first-line oral therapy for uncomplicated UTIs caused by Proteus mirabilis, if the organism is susceptible, with a standard course duration 1, 2
  • The European Urology Association suggests that fluoroquinolones (ciprofloxacin 500-750 mg twice daily or levofloxacin 750 mg daily) can be used as alternative oral agents if TMP-SMX resistance is suspected, but only if local resistance is <10% 2, 3
  • The American Urological Association recommends oral cephalosporins (cefpodoxime 200 mg twice daily or ceftibuten 400 mg daily) as second-line options for uncomplicated cystitis 1, 3

Uncomplicated Pyelonephritis

  • The Infectious Diseases Society of America recommends ciprofloxacin (500-750 mg twice daily for 7 days) or levofloxacin (750 mg daily for 5 days) as oral therapy for uncomplicated pyelonephritis, with a duration of 7-14 days 1, 3
  • The European Urology Association suggests that TMP-SMX (160/800 mg twice daily for 14 days) can be used if susceptible, with an initial IV dose of ceftriaxone 1 g or a consolidated 24-hour aminoglycoside dose before starting oral therapy 1, 3

Complicated UTIs

  • The European Urology Association recommends empiric parenteral therapy with amoxicillin plus aminoglycoside, second-generation cephalosporin plus aminoglycoside, or third-generation cephalosporin IV (ceftriaxone 1-2 g daily) for complicated UTIs, with a duration of 7-14 days 2, 5
  • The Infectious Diseases Society of America emphasizes the importance of addressing any underlying urological abnormality, which is mandatory for cure 5, 7

Key Practice Points

  • The European Urology Association recommends always obtaining urine culture and susceptibility testing before finalizing therapy, and tailoring empiric therapy based on local resistance patterns 1, 2, 5, 7
  • The Infectious Diseases Society of America suggests avoiding fluoroquinolones if patient used them in last 6 months or local resistance >10% 2, 3
