Lumbar Spine X-ray Ordering Guidelines

Indications for Lumbar Spine X-ray

  • The American College of Radiology recommends plain X-rays as "usually appropriate" (rating 9/9) for suspected vertebral fractures in patients with osteoporosis or on corticosteroids (>3 months) 1
  • X-rays are highly rated (8/9) for suspected fracture in patients with ankylosis, but may need additional imaging if negative 2
  • The American College of Radiology suggests X-rays for patients with persistent symptoms after 6 weeks of conservative management, or those with neurological deficits suggesting serious pathology 1

Appropriate Use of Lumbar Spine X-ray

  • X-rays of the axial skeleton are rated as "usually not appropriate" (1/9) for osteoporosis screening or BMD assessment, and DXA is preferred 1
  • Routine imaging for uncomplicated radiculopathy without red flags provides no clinical benefit, according to the American College of Radiology 3
  • For suspected inflammatory back pain or axial spondyloarthropathy with negative radiographs, MRI is preferred 2

Ordering and Interpretation Guidelines

  • The American College of Radiology recommends that X-rays include 2 views of the spine area of interest 1
  • The ultimate decision regarding the appropriateness of any radiologic examination must be made by the referring physician and radiologist based on the individual clinical circumstances 1
