Hypercalcemia of Malignancy Diagnosis and Management

Diagnostic Approach

  • The American College of Chest Physicians recommends measuring intact PTH (iPTH) to distinguish PTH-dependent from PTH-independent causes of hypercalcemia, with PTH expected to be suppressed in hypercalcemia of malignancy 1
  • Measuring PTHrP after finding suppressed PTH is the logical next step, as PTHrP-mediated hypercalcemia is the most common mechanism of malignancy-associated hypercalcemia, accounting for the majority of cases 1

Laboratory Evaluation

  • A complete diagnostic panel should include measurements of PTHrP, 1,25-dihydroxyvitamin D, 25-hydroxyvitamin D, serum calcium, albumin, phosphorus, magnesium, creatinine, and BUN, with the American College of Chest Physicians recommending simultaneous measurement of these parameters 1, 3
  • The pattern of suppressed iPTH, elevated PTHrP, low or normal 1,25-dihydroxyvitamin D, and normal or low 25-hydroxyvitamin D is most commonly seen in squamous cell carcinomas, neuroendocrine tumors, and various other solid malignancies 1

Prognostic Implications and Treatment

  • The median survival after discovery of hypercalcemia of malignancy in lung cancer patients is approximately 1 month, making this a medical emergency requiring prompt diagnosis and treatment, according to the American College of Chest Physicians 1
  • The American College of Chest Physicians recommends initiating IV hydration and bisphosphonates immediately in patients with severe hypercalcemia or symptomatic hypercalcemia, without delaying treatment while awaiting PTHrP results 1, 4

Monitoring and Pitfalls

  • PTHrP levels do not consistently decrease with successful calcium-lowering treatment, and should not be used to monitor treatment response, instead monitoring serum calcium, renal function, and electrolytes to assess treatment effectiveness 4
  • The American College of Chest Physicians recommends measuring both 25-hydroxyvitamin D and 1,25-dihydroxyvitamin D, as the relationship between these two provides critical diagnostic information, particularly for distinguishing PTHrP-mediated from calcitriol-mediated hypercalcemia 2
