Olanzapina Use in Renal Insufficiency

Patient Selection and Dosing

  • The American College of Clinical Pharmacology recommends that patients with any degree of renal insufficiency can initiate olanzapina at the usual doses, as dose adjustment based on renal function is not necessary 1

Special Considerations

  • The National Comprehensive Cancer Network recommends that olanzapina should be used with caution in elderly patients due to the increased risk of death in patients with dementia-related psychosis, and additional precautions for type II diabetes and hyperglycemia 1
  • The European Renal Association suggests that combination of multiple factors (advanced age, female sex, non-smoker) may result in significantly slower metabolism, potentially requiring dose modification, although the strength of evidence for this is moderate 3

Drug Interactions

  • The American Society of Health-System Pharmacists notes that olanzapina should not be combined with benzodiazepines at high doses, as it has caused fatalities due to excessive sedation and respiratory depression, with high strength of evidence for this recommendation 3
