Fluid Management for NPO Patients

Preferred Fluids

  • The American Heart Association recommends IV fluids as normal saline at 75-100 mL/h for NPO patients, specifically avoiding dextrose-containing solutions, to maintain normovolemia and prevent hypotension and cerebral hypoperfusion 1, 2
  • Normal saline at 75-100 mL/h is the standard maintenance fluid for NPO patients, as it helps prevent hypotension and cerebral hypoperfusion 1, 2

Risks of Dextrose-Containing Solutions

  • Glucose can have detrimental effects in acute brain injury of all types, making D5W and D1/2 NS inappropriate choices 1
  • Patients with diabetes or stress hyperglycemia require insulin management when NPO, not glucose-containing IV fluids 3, 4
  • Basal insulin or basal-plus-bolus correction insulin is the preferred treatment for NPO patients with diabetes, not glucose infusions 4

Clinical Algorithm

  • For standard NPO patients, use 0.9% normal saline at 75-100 mL/h as first-line maintenance fluid, and monitor intake and output closely 1, 2
