Treatment for Sphenoid Sinusitis
Clinical Presentation and Diagnosis
- Sphenoid sinusitis presents with characteristic permanent retro-orbital headache that radiates to the vertex, with intensity and focus that may simulate pain caused by intracranial hypertension, and is often accompanied by purulent discharge on the posterior pharyngeal wall 1, 2
Antibiotic Treatment
- The American College of Physicians recommends first-line treatment including amoxicillin-clavulanate, second/third generation cephalosporins, or fluoroquinolones active against pneumococci for sphenoid sinusitis, with consideration for surgical intervention if complications develop or medical therapy fails 1, 2
- For patients with beta-lactam allergies, pristinamycin is recommended as an alternative treatment option 1, 2
- The standard duration of antibiotic treatment is 7-10 days, although certain cephalosporins have shown effectiveness with 5-day courses 1, 2
Adjunctive Therapy
- Corticosteroids may be beneficial if given for a short period as adjuvant therapy in acute hyperalgic sinusitis, while NSAIDs at anti-inflammatory doses have not demonstrated proven efficacy in sinusitis 1, 2
- Supportive measures include adequate hydration, analgesics, warm facial packs, and sleeping with the head elevated 3
Surgical Management
- Indications for immediate surgical management include development of neurological complications, such as meningeal syndrome or visual disturbances 1, 2
