Treatment of Foreskin Inflammation
Initial Assessment and Etiology
- The British Journal of Dermatology recommends determining whether the inflammation is infectious (bacterial or fungal) or inflammatory/dermatologic before initiating treatment, looking for signs such as purulent discharge, fever, or systemic infection 2, 3
- Inflammatory/dermatologic conditions such as lichen sclerosus, Zoon's balanitis, or non-specific inflammation should be considered, as per the British Journal of Dermatology 1
Primary Treatment: Topical Corticosteroids
- The British Journal of Dermatology states that ultrapotent topical corticosteroids are the cornerstone of treatment for inflammatory foreskin conditions 1
- The recommended regimen is clobetasol propionate 0.05% ointment applied once daily at night for 4 weeks, then alternate nights for 4 weeks, and then twice weekly for 4 weeks, as per the British Journal of Dermatology 1
- Approximately 60% of patients achieve complete symptom remission with this treatment, and hyperkeratosis, ecchymoses, fissuring, and erosions should resolve, according to the British Journal of Dermatology 1
When Infection is Suspected
- If yeast infection is suspected, the Centers for Disease Control and Prevention (CDC) recommends clotrimazole 1% cream applied twice daily for 7-14 days, or miconazole 2% cream applied twice daily for 7 days, as stated in the MMWR Recommendations and Reports 2
- For bacterial infection, the CDC recommends ceftriaxone 250 mg IM once plus doxycycline 100 mg orally twice daily for 10 days, which covers both gonorrhea and chlamydia, as per the MMWR Recommendations and Reports 3
Adjunctive Measures
- The British Journal of Dermatology suggests applying white soft paraffin ointment every 4 hours to protect inflamed tissue 4
Follow-Up and Reassessment
- The British Journal of Dermatology recommends reassessing at 12 weeks after initiating clobetasol therapy, and long-term maintenance with clobetasol 0.05% as needed is safe and does not increase risk of skin atrophy or malignancy with proper monitoring 1
