Combination Therapy for Respiratory Conditions

Rationale for Combination Therapy

  • The American Thoracic Society suggests that prednisone (systemic corticosteroid) and budesonide (inhaled corticosteroid) can be used together to manage moderate to severe respiratory conditions, as they work through similar anti-inflammatory mechanisms but target different areas of the respiratory system 1, 3
  • The European Respiratory Society recommends that oral prednisone provides systemic anti-inflammatory effects throughout the body, while nebulized budesonide delivers medication directly to the airways, making this combination particularly beneficial during acute exacerbations of respiratory conditions 1, 4

Clinical Applications

  • The National Asthma Education and Prevention Program suggests that in acute severe asthma or COPD exacerbations, the combination of prednisone and budesonide provides more comprehensive anti-inflammatory coverage than either medication alone 1

Administration Guidelines

  • The American College of Chest Physicians recommends using a mouthpiece rather than a mask when administering nebulized budesonide to prevent facial deposition of the medication 1, 5
  • The European Respiratory Society suggests rinsing your mouth after using nebulized budesonide to prevent oral thrush 5
  • The American Thoracic Society recommends continuing regular preventative treatment alongside nebulizer therapy if prescribed 1, 4

Important Considerations

  • The American College of Chest Physicians suggests that if your nebulizer breaks down, you should use your hand-held inhaler until you can get help 1, 6
