Treatment of Restless Arm and Leg Symptoms in Elderly Patients

Initial Assessment and Treatment

  • The American Geriatrics Society recommends checking serum ferritin and transferrin saturation in elderly patients before initiating pharmacological therapy for restless legs syndrome, and supplementing with oral ferrous sulfate if ferritin ≤75 ng/mL or transferrin saturation <20% 3
  • Iron deficiency plays a critical role in RLS pathophysiology through impaired dopamine transport in the substantia nigra, and the threshold for iron supplementation in RLS is higher than general population guidelines because brain iron deficiency contributes to RLS even when serum iron appears normal 1, 2, 3
  • For patients who fail oral iron therapy, consider IV ferric carboxymaltose as a strong alternative with moderate certainty of evidence 1, 2

Pharmacological Treatment

  • The American College of Physicians recommends alpha-2-delta ligands (gabapentin, pregabalin, or gabapentin enacarbil) as first-line pharmacological treatment for restless legs syndrome in elderly patients, with strong evidence for efficacy and a better safety profile than dopamine agonists regarding fall risk 1, 2
  • Gabapentin dosing should start with 300 mg orally three times daily and titrate upward by 300 mg/day every 3-7 days until reaching maintenance dose of 1800-2400 mg/day divided three times daily, with common side effects including somnolence and dizziness 1
  • Pregabalin is an alternative with twice-daily dosing and potentially superior bioavailability compared to gabapentin, and may be preferred in elderly patients who struggle with three-times-daily dosing schedules 1, 2

Special Considerations

  • The Journal of the American Geriatrics Society recommends monitoring for signs of leg discomfort, excessive motor activity, and symptoms that occur only in evening/night or worse at those times in cognitively impaired elderly patients who are unable to articulate symptoms 3
  • The American College of Physicians recommends watching for augmentation if dopamine agonists were used, and switching to alpha-2-delta ligand (gabapentin/pregabalin) and tapering the dopamine agonist once new medication reaches therapeutic dose if augmentation occurs 3, 4
  • Recheck ferritin every 6-12 months as brain iron deficiency may persist despite symptom improvement 1, 4

Critical Pitfalls to Avoid

  • Benzodiazepines (like clonazepam) are explicitly NOT recommended for RLS treatment due to insufficient evidence of efficacy on validated outcomes, and they cause decreased cognitive performance in elderly patients—a particularly dangerous combination 1, 2
