Management of Escitalopram-Induced SIADH

Clinical Course and Management

  • The European Society for Medical Oncology guidelines recommend that discontinuation of escitalopram is essential for resolution of SIADH-induced hyponatremia, with medication withdrawal being the cornerstone of management 1, 2
  • Discontinuation of escitalopram typically leads to improvement in hyponatremia within days to weeks 1, 2

Acute Management Algorithm

  • The European Society for Medical Oncology guidelines suggest implementing fluid restriction to 1 L/day for mild-moderate cases, administering 3% hypertonic saline if severe symptoms are present, and never exceeding 8 mmol/L correction in 24 hours to prevent osmotic demyelination syndrome 2, 3
  • Adding oral salt supplementation may be necessary if fluid restriction alone is insufficient 2

High-Risk Populations

  • No specific guideline recommendations are provided for high-risk populations, however, the FDA warns that elderly patients and those on diuretics face substantially elevated risk for developing escitalopram-induced SIADH, and regular electrolyte monitoring is mandatory in these patients 1, 2
