Levofloxacin Dosing for Community-Acquired Pneumonia
Recommended Dosing Regimens
- For severe Community-Acquired Pneumonia (CAP) in hospitalized patients not requiring ICU, the Infectious Diseases Society of America recommends 750 mg IV/oral once daily for 5-7 days 1, 2
- For severe CAP in ICU or intermediate care, the Infectious Diseases Society of America suggests 750 mg IV once daily or 500 mg IV twice daily, often in combination with other antibiotics 1, 3
- For empiric treatment of CAP, the European Respiratory Society recommends levofloxacin as monotherapy in patients without risk factors for Pseudomonas aeruginosa 1, 2
- For patients with risk factors for P. aeruginosa, the Infectious Diseases Society of America recommends combining levofloxacin with antipseudomonal beta-lactams 1
Pathogen-Specific Dosing
- For Legionella species, the American Thoracic Society recommends 750 mg once daily, as levofloxacin has the most clinical data for Legionella treatment among fluoroquinolones 1
- For Mycoplasma pneumoniae, the American Academy of Family Physicians recommends 750 mg once daily 4
Duration of Therapy
- The Infectious Diseases Society of America recommends that treatment should generally not exceed 8 days in responding patients 1
- Biomarkers, particularly procalcitonin (PCT), may guide shorter treatment duration, according to the European Respiratory Society 1
Monitoring Response
- The Infectious Diseases Society of America recommends monitoring response to treatment using simple clinical criteria including body temperature, respiratory and hemodynamic parameters 1
- These same parameters should be used to determine suitability for hospital discharge, according to the American College of Chest Physicians 1
