Management of Elevated CK in Patients on Antipsychotics
Diagnostic Approach
- The American College of Cardiology recommends a comprehensive muscle enzyme panel, including aldolase, AST, ALT, LDH, troponin, and TSH, to differentiate between antipsychotic-induced myopathy, drug-drug interaction effects, and exercise-induced CK elevation 1, 2, 3
- A thorough evaluation should include troponin and ECG to exclude myocardial involvement, which can be life-threatening with antipsychotic-induced myopathy 1, 2, 3
- TSH, free T4, and free T3 should be assessed, as hypothyroidism predisposes to myopathy and can present with elevated CK and muscle cramps 1
Medication Management
- Benztropine should be discontinued immediately, as it may be contributing to the patient's presentation 1, 2
- If CK rises to 3-10× ULN or weakness develops, quetiapine and aripiprazole should be held immediately, and prednisone 0.5-1 mg/kg daily should be initiated 2, 3
- If CK exceeds 10× ULN or rhabdomyolysis develops, immediate hospitalization, permanent discontinuation of antipsychotics, and initiation of prednisone 1 mg/kg or methylprednisolone 1-2 mg/kg IV are recommended 2, 3
Monitoring and Follow-up
- Serial CK monitoring every 2-3 days initially, then weekly, is recommended until normalization 1, 2, 3
- Renal function and electrolytes should be reassessed if CK rises significantly 1
- Reassess for weakness development at each follow-up, as this changes management tier from observation to active treatment 1
Red Flags and Urgent Escalation
- Progressive proximal muscle weakness, dysphagia, dysarthria, dysphonia, or dyspnea require immediate intervention 4, 3
- Cardiac symptoms or troponin elevation mandate permanent drug discontinuation 3
- CK >1000 IU/L is a rhabdomyolysis threshold with acute kidney injury risk 1
Addressing Elevated Inflammatory Markers
- Concurrent inflammatory or autoimmune condition should be considered if symptoms persist beyond 4 weeks 5, 6
- Adult-onset Still's disease, though unlikely, should be considered if fever, rash, or sore throat are present 6
- Inflammatory myositis requires EMG, muscle MRI, or biopsy if weakness develops 4, 1
