Steroid Regimen for Hypophysitis
Severity-Based Treatment Algorithm
- For patients with severe symptoms, such as severe headache, visual disturbances, or hypoadrenalism, the American Society of Clinical Oncology recommends initiating intravenous methylprednisolone 1 mg/kg daily 1, 2, 3
- The European Society for Medical Oncology suggests that patients with moderate symptoms without mass effect should use oral prednisolone 0.5-1 mg/kg daily 1
- High-dose steroids are essential for neurological symptoms but do not reverse hormonal deficiencies, which require lifelong hormone replacement therapy, as stated by the American College of Endocrinology 2, 3, 5
- For severe headache with visual disturbances, hypotension, or severe electrolyte disturbances from hypoadrenalism, immediate intervention is required, including initiation of IV methylprednisolone 1 mg/kg daily after obtaining pituitary axis blood work 1, 2
- The American Society of Clinical Oncology recommends withholding immune checkpoint inhibitor therapy immediately in cases of severe symptoms 1, 4
Critical Management Principles
- Physiologic hormone replacement is the cornerstone of management, not high-dose steroids, according to the European Society for Medical Oncology 2, 3, 5
- The American College of Endocrinology suggests starting hydrocortisone (20/10/10 mg daily) before thyroid hormone to prevent adrenal crisis 1, 2, 3
- Lifelong hormone replacement is required in most cases, as hormonal deficiencies rarely recover, as stated by the American Society of Clinical Oncology 2, 3, 5
Steroid Tapering Protocol
- The American Society of Clinical Oncology recommends tapering steroids gradually under close monitoring, with a goal of maintaining a dose of 5 mg prednisolone 1
- For Grade 2 episodes, steroids should be weaned over 2-4 weeks, and for Grade 3/4 episodes, over 4 weeks 1
