Management of Autoantibodies in Connective Tissue Diseases
Initial Clinical Assessment
- The American College of Rheumatology recommends evaluating patients with positive ENA antibody screen, positive SS-A (Ro-60), positive Ro-52, and equivocal SS-B antibodies for symptoms of Sjögren's syndrome, including dry eyes, dry mouth, fatigue, and musculoskeletal pain 1
- Patients with positive ENA antibody screen, positive SS-A (Ro-60), positive Ro-52, and equivocal SS-B antibodies should be assessed for symptoms of SLE, including rash, joint pain, photosensitivity, and constitutional symptoms 1
Complete Autoimmune Profile
- The European League Against Rheumatism recommends completing the autoantibody profile with additional testing, including anti-dsDNA antibodies, anti-Smith antibodies, complete ANA panel with pattern and titer, rheumatoid factor, and complement levels (C3, C4) 1, 2, 3, 4, 5
- Anti-dsDNA antibodies should be tested using both SPA and CLIFT methods for optimal sensitivity and specificity 2, 3
- Anti-Smith antibodies are highly specific for SLE and should be included in the autoantibody profile 4
Objective Clinical Evaluation
- For suspected Sjögren's syndrome, the American College of Rheumatology recommends performing ophthalmologic evaluation with Schirmer's test and ocular surface staining, conducting oral examination with assessment of salivary flow, and considering minor salivary gland biopsy if other findings support Sjögren's syndrome 1
- For suspected SLE or other connective tissue diseases, patients should undergo complete blood count with differential, comprehensive metabolic panel with renal function, urinalysis, and urine protein/creatinine ratio to evaluate for renal involvement 1, 5
- Total IgG and subclass levels should be measured, particularly in patients who may need immunosuppressive therapy 5
Special Considerations
- Women of childbearing age with positive SS-A antibodies should be counseled about the risk of neonatal lupus and congenital heart block, and considered for hydroxychloroquine to reduce the risk of these complications in future pregnancies 1
