Immediate Treatment for Starvation Ketoacidosis
Pathophysiology and Diagnosis
- The American Diabetes Association notes that starvation ketoacidosis is distinguished from diabetic ketoacidosis (DKA) by mildly elevated plasma glucose or even hypoglycemia, and serum bicarbonate typically not lower than 18 mEq/L 1
Treatment Algorithm
- The American College of Physicians recommends aggressive fluid resuscitation with isotonic saline (0.9% NaCl) at a rate of 15-20 ml/kg/hour and oral or intravenous glucose administration of 150-200g of carbohydrate per day to reverse the ketosis 1, 2
- The American Association of Clinical Endocrinologists suggests providing 150-200g of carbohydrate daily (45-50g every 3-4 hours) for carbohydrate administration 2
- The American College of Emergency Physicians recommends checking serum electrolytes, particularly potassium, sodium, and phosphate, and replacing electrolytes as needed based on laboratory values 1
Monitoring for Resolution
- The American College of Physicians indicates that treatment success is indicated by resolution of acidosis (pH >7.3), serum bicarbonate ≥18 mEq/L, anion gap ≤12 mEq/L, and improvement in clinical symptoms 1, 4
Common Pitfalls to Avoid
- The American Diabetes Association warns that inadequate carbohydrate replacement, failure to provide sufficient carbohydrates (150-200g daily), may lead to persistent ketosis 2
- The American College of Physicians advises that failure to monitor electrolytes and acid-base status may lead to complications 4
