Maximum Recommended Dose of Venofer (Iron Sucrose)

Dosing Guidelines for Iron Sucrose

  • The FDA recommends a maximum single dose of 200 mg of Venofer (iron sucrose), with a maximum weekly dose of 500 mg 1, 2
  • The concentration of elemental iron in Venofer is 20 mg/ml, according to the FDA guidelines 1
  • For single dose administration, the minimum infusion time for the maximum dose is 15 minutes, and the minimum injection time for the maximum dose is 2-5 minutes, as recommended by the FDA 1
  • For weekly dosing, individual doses above 300 mg are not recommended, as stated by the National Comprehensive Cancer Network 3, 4, 5

Administration Methods

  • Iron sucrose can be administered by IV infusion, with a dose of 200 mg over 60 minutes, or by IV push, with a dose of 200 mg over 2-5 minutes, as recommended by the National Comprehensive Cancer Network 3, 4
  • Repeated dosing every 2-3 weeks is also recommended by the National Comprehensive Cancer Network 3, 4

Safety Considerations

  • Test doses are not required for iron sucrose, unlike iron dextran, according to the National Comprehensive Cancer Network 3, 4
  • However, test doses (25 mg slow IV push) are strongly recommended if patients have previous sensitivities to iron dextran or other IV iron preparations, or multiple drug allergies, as stated by the National Comprehensive Cancer Network 3, 4
  • Patients with active infection should not receive IV iron therapy, as recommended by the National Comprehensive Cancer Network 3, 4

Clinical Applications

  • In cancer patients receiving ESAs, individual doses above 300 mg are not recommended, and the maximum total dose is 1000 mg, as stated by the National Comprehensive Cancer Network 3, 4
