Plaque Psoriasis Treatment Guidelines

Introduction to Treatment Options

  • The American Academy of Dermatology recommends combination calcipotriene plus betamethasone dipropionate applied once daily for 4 weeks as first-line treatment for mild plaque psoriasis (<5% body surface area), providing superior efficacy and safety compared to monotherapy 1
  • For moderate-to-severe disease (≥5% BSA), the American Academy of Dermatology recommends initiating biologic therapy with adalimumab or infliximab as first-line systemic treatment 1

Treatment Algorithm Based on Disease Severity

  • For mild psoriasis (<5% BSA), the National Psoriasis Foundation suggests using combination calcipotriene/betamethasone dipropionate as a single product once daily for 4 weeks, achieving maximal improvement in the majority of patients 1
  • Maintenance regimens after initial 4 weeks include applying vitamin D analogues twice daily on weekdays combined with high-potency corticosteroids twice daily on weekends, with long-term use up to 52 weeks being safe and recommended 1
  • For scalp psoriasis, the American Academy of Dermatology recommends using calcipotriene foam or calcipotriene plus betamethasone dipropionate gel for 4-12 weeks 1
  • Generic clobetasol propionate 0.05% solution twice daily is highly effective and among the least expensive options for scalp psoriasis 1

Biologic Therapy Options

  • Adalimumab is a preferred TNF inhibitor, with dosing at 80 mg initial dose, then 40 mg one week later, then 40 mg every 2 weeks, achieving PASI 75 in 71% of patients at week 16 versus 7% with placebo 1
  • Infliximab is an alternative first-line biologic, with dosing at 5 mg/kg IV at weeks 0, 2, and 6, then every 8 weeks, achieving PASI 75 in 80% of patients 1, 2
  • Etanercept is an alternative option, with dosing at 50 mg subcutaneously twice weekly for 12 weeks, then 50 mg once weekly, achieving PASI 75 in 48-66% at week 12 1, 2

Combination Strategies and Special Clinical Scenarios

  • The American Academy of Dermatology recommends adding high-potency topical corticosteroids with or without vitamin D analogues to augment biologic efficacy 1, 2
  • Combining biologics with methotrexate or acitretin may also augment efficacy 1, 2
  • For psoriatic arthritis, adalimumab or infliximab are strongly recommended regardless of skin disease severity, as they treat skin and joint symptoms simultaneously and inhibit radiographically detected joint damage 1, 2

Critical Safety Considerations and Common Pitfalls to Avoid

  • The National Psoriasis Foundation advises against using systemic corticosteroids (prednisone) due to poor long-term efficacy and significant safety concerns 3
  • Topicals alone are insufficient for >5% BSA involvement, and biologics have a high benefit-to-risk ratio and should not be delayed 1, 2
  • Acitretin should not be used in women of childbearing potential due to FDA requirements and concerns including hepatotoxicity and hypertriglyceridemia 4
