Pneumococcal Vaccination Protocol

General Recommendations for Adults

  • The Centers for Disease Control and Prevention recommends a single dose of PCV20 for all adults aged 65 and older who have not previously received a pneumococcal conjugate vaccine or whose vaccination history is unknown 1, 2
  • For adults aged 19 to 64 with certain underlying medical conditions, a single dose of PCV20 is recommended if they have not previously received a pneumococcal conjugate vaccine 1, 4

Specific Protocols for Particular Populations

  • For immunocompromised adults who have received only PCV13, administer PPSV23 at least 8 weeks later, followed by a second dose of PPSV23 at least 5 years after the first dose of PPSV23 4
  • For recipients of hematopoietic stem cell transplants, administer 3 doses of PCV20, 4 weeks apart, starting 3 to 6 months after the transplant, followed by a fourth dose of PCV20 at least 6 months after the third dose 1, 3

Recommendations for Patients Who Have Already Received Pneumococcal Vaccines

  • For adults who have started their pneumococcal vaccination series with PCV13 but have not received all recommended doses of PPSV23, administer either a single dose of PCV20 or at least 1 dose of PPSV23 5
  • For adults who have received PPSV23 but no conjugate vaccine, administer PCV15 or PCV20 5

Important Clinical Considerations

  • Pneumococcal and influenza vaccines can be administered at the same visit to different anatomical sites 3
  • For patients who have received PCV15 but have not completed their series with PPSV23, a dose of PCV20 can be used if PPSV23 is not available 3
