Switching from Caplyta 42 mg to Risperidone

Initial Dosing Strategy

  • The American Academy of Family Physicians suggests starting risperidone at a low dose of 0.5 mg twice daily (total 1 mg/day) for patients switching from Caplyta 42 mg to minimize withdrawal effects and ensure continuous symptom control 1, 2
  • For elderly patients or those with renal/hepatic impairment, consider an even lower starting dose of 0.25-0.5 mg once daily, as recommended by the American Family Physician 2, 3

Risperidone Dose Titration

  • The American Family Physician recommends titrating risperidone gradually to the target dose based on clinical response after Caplyta is discontinued 3
  • The maximum recommended dose of risperidone is typically 6 mg/day, with increased risk of extrapyramidal symptoms (EPS) above this dose, according to the British Journal of Psychiatry 2, 4
  • Allow 3-7 days between dose increases to assess response and tolerability, as suggested by the Mayo Clinic Proceedings 5

Special Considerations

  • Patients with a history of EPS with previous antipsychotics may require slower titration of risperidone, as recommended by the American Academy of Family Physicians 2
  • Risperidone is available as an orally disintegrating tablet (ODT) which may improve adherence during the transition period, according to the American Academy of Family Physicians 2

Potential Pitfalls to Avoid

  • Be aware that risperidone has a higher propensity for causing EPS than Caplyta, particularly at doses above 6 mg/day, as reported by the American Academy of Family Physicians 2
  • For patients with Parkinson's disease or Lewy body dementia, risperidone should be used with extreme caution due to the high risk of worsening motor symptoms, as recommended by the American Academy of Family Physicians 2
