Potassium Citrate Dosage for Kidney Stone Prevention

Dosing Guidelines

  • The American Urological Association recommends a dose of 30-80 mEq per day divided into 3-4 doses for patients with recurrent calcium stones and low urinary citrate to effectively prevent kidney stone formation 1
  • Potassium citrate is preferred over sodium citrate as the sodium load in the latter may increase urine calcium excretion 2
  • For uric acid stones, the recommended dosage is 30-80 mEq per day in three-to-four divided doses, with a target urinary pH of 6.0 4
  • For cystine stones, the recommended dosage is 30-80 mEq per day in three-to-four divided doses, with a target urinary pH of 7.0 and high fluid intake (at least 4 liters per day) 4, 5

Adjunctive Measures

  • Maintaining adequate fluid intake is essential to ensure sufficient urine volume 7
  • Restricting sodium intake (100 mEq/2,300 mg or less daily) can help enhance the efficacy of potassium citrate 8
  • Consuming ample fruits and vegetables can help counterbalance foods that confer an acid load 2
  • Thiazide diuretics can be considered in combination with potassium citrate for patients with hypercalciuria 9

Special Considerations

  • Patients with severe renal impairment should be monitored closely as they are at risk of developing hyperphosphatemia 10
  • Sodium citrate should be avoided as the sodium load may increase urine calcium excretion 2
