Levofloxacin Dosing in Liver Cirrhosis

Clinical Use of Levofloxacin in Cirrhosis

  • The European Association for the Study of the Liver recommends levofloxacin for various indications, including spontaneous bacterial peritonitis (SBP) prophylaxis and treatment of infections in cirrhotic patients 1, 2
  • The American Association for the Study of Liver Diseases suggests guidelines for management of SBP, mentioning fluoroquinolones, including ciprofloxacin, as alternatives to norfloxacin for SBP prophylaxis, indicating standard dosing is appropriate 3, 4
  • The European Association for the Study of the Liver recommends antibiotic prophylaxis, with quinolones being an option, in patients with cirrhosis presenting with gastrointestinal bleeding 5, 6

Monitoring and Special Considerations

  • Regular monitoring of renal function is essential, and patients should be watched for QT prolongation, especially when taking other medications that may prolong QT interval, as recommended by the Journal of Hepatology 5
  • The Clinical and Molecular Hepatology journal notes that increasing bacterial resistance to fluoroquinolones, particularly in patients with previous quinolone exposure, should be considered when prescribing levofloxacin 1, 2
  • The Journal of Hepatology recommends avoiding unnecessary use of proton pump inhibitors in cirrhotic patients on quinolones, as PPIs may increase the risk of SBP 2, 4
