Perioperative Management of Lovenox (Enoxaparin)

General Recommendations

  • The American College of Chest Physicians recommends administering the last preoperative dose of therapeutic Lovenox 24 hours before surgery to minimize bleeding risk 1, 2, 3
  • This timing allows for approximately 4-5 half-lives to elapse before surgery, minimizing residual anticoagulant effect 4
  • Resume therapeutic-dose Lovenox 48-72 hours after surgery when hemostasis is adequate for high bleeding risk procedures 3, 1
  • For low bleeding risk procedures, resume Lovenox at previous therapeutic dose 24 hours after surgery 4, 1

Rationale for 24-Hour Preoperative Discontinuation

  • Studies show that when Lovenox is discontinued only 12 hours before surgery, >90% of patients have detectable anticoagulant effect at the time of surgery 1
  • Approximately 34% of patients still have therapeutic anticoagulant levels when Lovenox is stopped only 12 hours preoperatively 1
  • The elimination half-life of Lovenox is 3-5 hours, which informs the pre-operative interruption timing 1

Special Considerations

  • For patients on warfarin requiring bridging with Lovenox, the same timing principles apply, with warfarin stopped 5 days before surgery and therapeutic Lovenox begun approximately 3 days before surgery 5, 2, 6, 4
  • Patients with renal impairment may require longer discontinuation periods due to delayed clearance of Lovenox 7, 4
  • Consider individual patient factors, including renal function, when determining exact timing 4

Post-Operative Resumption

  • For high bleeding risk procedures, resume therapeutic Lovenox 48-72 hours after surgery 3, 1
  • For low bleeding risk procedures, resume therapeutic Lovenox 24 hours after surgery 4, 1
  • Consider prophylactic doses initially if bleeding risk remains elevated 5, 4

Common Pitfalls to Avoid

  • Discontinuing Lovenox too close to surgery time (less than 24 hours) increases bleeding risk 1
  • Failing to account for renal function when determining discontinuation timing 7
  • Not considering the patient's thrombotic risk when planning perioperative management 4, 2
