Enterococcal Prostatitis Treatment Guidelines

  • The European Society of Cardiology recommends amoxicillin 200 mg/kg/day IV in 4-6 divided doses for 6 weeks as the preferred regimen for beta-lactam susceptible enterococcal prostatitis 1, 2
  • For resistant or intolerant cases, the Infectious Diseases Society of America strongly recommends linezolid 600 mg orally or IV every 12 hours for 4-6 weeks as first-line therapy for enterococcal prostatitis 6, 4, 5
  • High-dose daptomycin 8-12 mg/kg/day IV combined with ampicillin 200 mg/kg/day IV in 4-6 divided doses for 6-8 weeks is recommended for multidrug-resistant enterococcal infections 6, 1, 2

Critical Problems with the Proposed Regimen

  • The proposed amoxicillin dosing of 1 gram three times daily is only 25% of the recommended dose for enterococcal infections requiring 12 grams daily 3, 1
  • Three weeks of therapy is half the minimum recommended duration for complicated enterococcal infections 1, 2

Alternative Evidence-Based Regimens

  • The American Urological Association recommends amoxicillin 12 grams/day IV in 4-6 divided doses for 6 weeks as a first-line regimen for susceptible strains 1, 2
  • For resistant strains or beta-lactam intolerance, the Infectious Diseases Society of America recommends linezolid 600 mg PO/IV every 12 hours for 4-6 weeks 6, 4, 5

Common Pitfalls to Avoid

  • The European Society of Cardiology advises against using fluoroquinolones empirically for enterococcal prostatitis due to high resistance rates 4, 5
  • The American Urological Association recommends avoiding gentamicin-based regimens for prostatitis due to poor prostatic tissue penetration 1, 2
