Cefuroxime Axetil Pediatric Dosing Guidelines

Standard Dosing Guidelines

  • The recommended pediatric dose of cefuroxime axetil is 20-50 mg/kg/day divided into two doses every 12 hours, with a maximum of 500 mg per dose, as recommended by the American Academy of Pediatrics 1
  • The dose should not exceed adult dosing limits, according to the Infectious Diseases Society of America 2, 3
  • For severe infections, the parenteral cefuroxime dose is 100-200 mg/kg/day divided every 6-8 hours, with a maximum single dose of 1500 mg, as suggested by the American Academy of Pediatrics 1

Clinical Context and Indications

  • Cefuroxime axetil is appropriate for community-acquired pneumonia caused by Streptococcus pneumoniae with penicillin MIC <2.0 μg/mL, as recommended by the American Academy of Pediatrics 1
  • It is an alternative therapy to amoxicillin or amoxicillin/clavulanate for respiratory tract infections, according to the Infectious Diseases Society of America 1
  • Cefuroxime axetil is effective against beta-lactamase producing organisms, including Haemophilus influenzae, as suggested by the American Academy of Pediatrics 2, 3

Age-Specific Cautions

  • All infants <6 months with pneumonia should be hospitalized, regardless of antibiotic choice, as recommended by the American Academy of Pediatrics 1

Common Pitfalls to Avoid

  • The 20 mg/kg/day dose may be insufficient for serious infections; use 40-50 mg/kg/day for pneumonia, as recommended by the American Academy of Pediatrics 1
  • Oral cefuroxime axetil dosing (20-50 mg/kg/day) differs significantly from IV cefuroxime (100-200 mg/kg/day), as suggested by the American Academy of Pediatrics 1
  • In school-aged children (≥5 years), if Mycoplasma pneumoniae is suspected, a macrolide is preferred over cefuroxime, according to the American Academy of Pediatrics 1
