Management of Displaced and Angulated Mid-Shaft 5th Metatarsal Fractures
Initial Assessment and Management
- The American Academy of Orthopaedic Surgeons recommends providing appropriate immobilization and pain control at the initial presentation, as early fracture stabilization provides the most effective analgesia 1
- Obtaining three-view radiographs (AP, lateral, oblique) to confirm diagnosis is recommended in the initial assessment 1
Timing and Intervention
- The American Academy of Orthopaedic Surgeons suggests that isolated 5th metatarsal shaft fractures do not carry the same mortality/morbidity risks from delayed treatment as hip fractures or severe limb trauma, and can be seen by orthopedics within 1-2 weeks 2, 3
- In contrast to 5th metatarsal fractures, hip fractures in elderly patients require surgical timing within 24-48 hours to reduce mortality, as emphasized by the American Academy of Orthopaedic Surgeons 2, 4, 5
- Severe limb trauma with diaphyseal fractures (femur/tibia with systemic complications) also requires urgent surgical intervention within 24-48 hours, as highlighted by Anaesthesia 3, 6
