Clinical Applications of Venous Blood Gas Analysis
Primary Clinical Applications
- The American Thoracic Society recommends that venous blood gas (VBG) analysis can be used to assess acid-base status and ventilation in patients with COPD or risk factors for hypercapnic respiratory failure, where repeated sampling is needed 1, 2
Important Limitations and Caveats
- The European Respiratory Society guidelines suggest that initial blood gas measurement should still be arterial in patients with shock, hypotension, or critical illness, as VBG cannot replace ABG for oxygenation assessment 3, 1
Practical Algorithm for VBG Use
- For hemodynamically stable patients without severe shock, the American College of Chest Physicians recommends using VBG + pulse oximetry for initial acid-base and ventilation assessment, and proceeding to ABG only if oxygenation assessment is critical or patient deteriorates 1, 2
- For critically ill or shocked patients, the British Thoracic Society guidelines recommend obtaining arterial sample initially, and VBG may be used for subsequent monitoring once stability is achieved 3, 1
