Evaluation and Management of Kidney Cysts

Initial Evaluation Approach

  • The American College of Radiology recommends starting with ultrasound as the initial imaging modality to detect and characterize renal cysts due to its non-invasive nature, lack of radiation exposure, and cost-effectiveness 1
  • For patients with few cysts and normal kidney function, proceed with kidney MRI (or CT with/without contrast) to apply Bosniak classification criteria, as recommended by the Kidney International guidelines 2
  • For patients with multiple cysts and/or discordant imaging/GFR, or atypical renal or extrarenal findings, obtain kidney MRI and consider genetic testing (PKD1, PKD2, HNF1B genes using ACMG criteria) to establish diagnosis, according to the Kidney International guidelines 2, 3

Management Based on Bosniak Classification

  • The Kidney International guidelines recommend no intervention or follow-up for asymptomatic simple cysts regardless of size, as they have a ~0% malignancy risk 1
  • For symptomatic simple cysts causing pain or hypertension, treatment success is defined by symptom relief rather than volume reduction, as stated in the Praxis Medical Insights guidelines 1
  • For Bosniak IIF (moderately complex cysts), perform active surveillance with repeat imaging in 6-12 months, due to a ~10% malignancy risk 1
  • For Bosniak III and IV (complex cysts), intervention is recommended when anticipated oncologic benefits outweigh risks, with a ~50% risk for Bosniak III and ~100% risk for Bosniak IV, according to the Praxis Medical Insights guidelines 1

Special Clinical Scenarios

  • The Kidney International guidelines recommend using age-specific ultrasound criteria to diagnose or exclude ADPKD, with specific criteria for different age groups 4
  • For patients with positive family history of ADPKD, use kidney MRI to confirm diagnosis if ultrasound findings are equivocal, as recommended by the Kidney International guidelines 4
  • Genetic testing is particularly helpful in situations such as few kidney cysts present, variable intrafamilial disease severity, or discordant imaging and glomerular filtration rate, according to the Kidney International guidelines 2, 4
  • The Kidney International guidelines recommend using serum creatinine with eGFRcr as the initial assessment of kidney function, and measuring cystatin C and calculating eGFRcr-cys if more accurate GFR assessment is needed 5

Critical Pitfalls to Avoid

  • Do not assume chronicity based on a single abnormal eGFR or ACR, as this could represent acute kidney injury or acute kidney disease rather than chronic kidney disease, as stated in the Kidney International guidelines 3
  • A solitary cyst in childhood requires follow-up imaging, as it may indicate ADPKD in children with positive family history, according to the Praxis Medical Insights guidelines 1
  • Simple renal cysts may be associated with hypertension, particularly when multiple cysts are present, as noted in the Praxis Medical Insights guidelines 1
