Approach to Mildly Elevated Liver Enzymes

Introduction to Autoimmune Hepatitis Management

  • The American Association for the Study of Liver Diseases recommends that immunosuppressive treatment should NOT be initiated in patients with mildly elevated liver enzymes and normal AST without clear evidence of autoimmune hepatitis, but close monitoring every 3-6 months is mandatory to detect disease progression 1, 2

Treatment Thresholds for Autoimmune Hepatitis

  • The American Association for the Study of Liver Diseases suggests that treatment is indicated for patients with AST or ALT >10-fold ULN 1
  • The American Association for the Study of Liver Diseases also recommends treatment for patients with AST or ALT ≥5-fold ULN with serum γ-globulin ≥2-fold ULN 1
  • Histological features of bridging necrosis or multilobular necrosis are also definite indications for treatment 1

Diagnostic Workup and Monitoring

  • The European Association for the Study of the Liver recommends checking autoimmune markers, including ANA, SMA, anti-LKM1 antibodies, serum IgG and γ-globulin levels 3, 2
  • The American Association for the Study of Liver Diseases suggests that liver biopsy is essential for AIH diagnosis and should be performed before initiating treatment, especially if transaminases remain persistently elevated, autoantibodies are positive, or IgG is elevated 1, 6, 7
  • Close follow-up every 3-6 months with AST, ALT, bilirubin, γ-globulin or IgG levels, and autoantibody titers is recommended 1, 2

Triggers for Treatment Initiation

  • The American Association for the Study of Liver Diseases recommends reconsidering treatment if ALT rises to >5x ULN with γ-globulin >2x ULN 1
  • Treatment should also be considered if ALT rises to >10x ULN, regardless of other parameters 1
  • Development of symptoms, such as fatigue, jaundice, or right upper quadrant pain, is also a trigger for treatment initiation 1, 2
  • Liver biopsy showing interface hepatitis with moderate-to-severe activity or evidence of progressive fibrosis on repeat biopsy or imaging are also indications for treatment 1, 3, 6, 7
