Upadacitinib: Evidence‑Based Recommendations and Safety Considerations

Evidence Base

  • International guidelines and global clinical trials that included diverse populations form the basis for upadacitinib recommendations in India, as no India‑specific trial data exist. 1

Ulcerative Colitis – Induction and Maintenance Dosing

  • For induction of remission in ulcerative colitis, administer upadacitinib 45 mg orally once daily for 8 weeks. 1
  • For maintenance after achieving a clinical response, use upadacitinib 15 mg or 30 mg orally once daily, selecting the dose based on disease severity and patient risk profile. 1
  • High‑certainty evidence demonstrates a large benefit of both induction and maintenance regimens in achieving remission, including in patients previously exposed to biologic therapy. 1

Absolute Contraindications

  • Active malignancy is an absolute contraindication to upadacitinib therapy. 1
  • Patients aged ≥ 50 years with a history of major adverse cardiovascular events or stroke who also have cardiovascular risk factors should not receive upadacitinib. 1

Risk Stratification (European Medicines Agency Guidance)

  • The European Medicines Agency advises extreme caution or avoidance of JAK inhibitors, including upadacitinib, in the following high‑risk groups:

  • In patients with VTE risk factors, cancer risk, or major cardiovascular risk, the lowest effective maintenance dose should be used (e.g., 15 mg for ulcerative colitis maintenance rather than 30 mg). 1

Safety Monitoring

  • Hyperlipidemia is a recognized adverse effect of upadacitinib; routine lipid panel monitoring is recommended. 1

Alternative Therapies for Ulcerative Colitis

Alternative Agent Typical Dosing (induction/maintenance) Reported Efficacy
Infliximab 5 mg/kg IV at weeks 0, 2, 6 then q8 weeks Standard biologic option
Adalimumab 160 mg SC day 0, 80 mg week 2, then 40 mg q2 weeks Standard biologic option
Vedolizumab 300 mg IV at weeks 0, 2, 6 then q8 weeks Standard biologic option
Golimumab 200 mg SC at week 0, 100 mg week 2 then 100 mg q4 weeks Standard biologic option
Filgotinib 200 mg orally once daily (induction) – 26 % remission at week 10 vs 15 % with placebo in biologic‑naïve patients Demonstrated efficacy in UC [1]

Smoking‑Related Risk

  • Current or long‑term former smokers have a significantly higher risk of adverse events when treated with upadacitinib. 1

Upadacitinib Dosing Guidelines

Standard Dosing by Indication

  • The standard dose of upadacitinib is 15 mg once daily for rheumatoid arthritis, as recommended by the FDA and EMA, with no dose adjustment needed for renal impairment 2, 3
  • For atopic dermatitis, the dose is 15 mg or 30 mg once daily, depending on disease severity, with the 30 mg dose demonstrating highest efficacy for reducing EASI scores 4
  • For ulcerative colitis, the induction dose is 45 mg once daily for 8 weeks, followed by a maintenance dose of 15 mg or 30 mg once daily after achieving clinical response 5

Dose Adjustments and Contraindications

  • No dose adjustment is needed for mild to moderate hepatic impairment, but upadacitinib is contraindicated in severe hepatic impairment (Child-Pugh C) 2, 3
  • No dose adjustment is required for any degree of renal impairment, distinguishing upadacitinib from other JAK inhibitors like baricitinib 2, 3

Critical Safety Considerations

  • The European Medicines Agency recommends caution when using JAK inhibitors in patients ≥65 years of age, those with cardiovascular risk factors, current or long-term past smokers, and those at increased risk of malignancy 5
  • Baseline screening is required for tuberculosis, hepatitis B, hepatitis C, and pregnancy, as well as complete blood count with differential, liver enzymes, renal function, and lipid panel 2, 4

Monitoring Protocol

  • Regular monitoring is required for CBC with differential and liver enzymes, lipid panel, and signs of VTE (chest pain, leg swelling, shortness of breath) 6, 4

Drug Interactions

  • No significant interactions have been reported with methotrexate when used in combination with upadacitinib 2, 3

Common Pitfalls to Avoid

  • The 30 mg dose should not be used in rheumatoid arthritis, as only the 15 mg dose is approved 2, 4, 5
  • Cardiovascular risk stratification should not be overlooked, as the ORAL surveillance study with tofacitinib showed increased MACE and malignancy in high-risk RA patients 6, 5, 7
