Management of Sinus Tachycardia with Diltiazem

Introduction to Sinus Tachycardia

  • The American College of Cardiology recommends Diltiazem as a second-line agent for managing sinus tachycardia, primarily when beta-blockers are contraindicated or ineffective 1, 2

Types of Sinus Tachycardia

  • Physiological sinus tachycardia is an appropriate response to physical, emotional, pathological, or pharmacological stress 3
  • Inappropriate sinus tachycardia (IST) is a persistent increase in heart rate at rest without identifiable cause or disproportionate to stress level 4, 3

Treatment Approach

  • The American College of Cardiology suggests that beta-blockers are the first-line treatment for symptomatic sinus tachycardia, particularly effective for stress-related and anxiety-related tachycardia 2, 3
  • Beta-blockers are more effective than calcium channel blockers for controlling ventricular response 5
  • Diltiazem should be considered when beta-blockers are contraindicated or ineffective 1, 2
  • Diltiazem is particularly useful in patients with hyperthyroidism-related tachycardia when beta-blockers are contraindicated 6

Treatment Algorithm

  • Identify and treat underlying causes of sinus tachycardia (anemia, dehydration, pain, hyperthyroidism, etc.) 4, 3
  • For symptomatic patients, start with beta-blockers as first-line therapy 2, 3
  • Consider diltiazem when beta-blockers are contraindicated or ineffective 1, 2, 6
