Adverse Effects of Excessive Bronchodilator Use

Cardiovascular Effects

  • The American Heart Association notes that excessive use of bronchodilators can cause serious cardiovascular effects, including tachycardia, arrhythmias, hypertension, myocardial ischemia, and in rare cases takotsubo cardiomyopathy 1, 2, 3
  • Tachycardia is a common side effect, with heart rate increases averaging 13 beats per minute with some agents like metaproterenol 3
  • Palpitations occur frequently with salbutamol (albuterol) use 3
  • Premature ventricular contractions have been documented with fenoterol and albuterol 3
  • Risk of myocardial ischemia increases in patients with underlying cardiac disease 3, 4
  • Rare but serious cardiac complications include takotsubo cardiomyopathy associated with repetitive albuterol use 3
  • Delivery method impacts cardiovascular effects - metered dose inhalers (MDIs) cause less tachycardia than nebulizers (6.47 beats/min less) 3
  • Intravenous administration causes more rapid heart rate increases than inhaled routes 5, 6

Metabolic Effects

  • Hypokalemia is consistently documented with short-acting β-agonists 3
  • Mean decreases in serum potassium typically range from 0.52-0.54 mmol/L 3
  • Hyperglycemia can occur with excessive use 7
  • Lactic acidosis has been reported 7

Neurological and Musculoskeletal Effects

  • Headache is commonly reported 7
  • Dizziness can occur with excessive use 3
  • Nervousness and weakness may be experienced 7

Respiratory Effects

  • Severe bronchospasm has been reported after exposure to albuterol inhaler and nebulizer treatment 3

Other Adverse Effects

  • Unilateral mydriasis has been reported with ipratropium when it comes in contact with the eye 3
  • Altered sleep and behavior patterns can develop, particularly with methylxanthines like theophylline 6
  • Gastroesophageal reflux may worsen with methylxanthines 6
  • Regular cleaning of HFA actuators prevents drug plugging and improper dosing 7

Prevention of Adverse Effects

  • The American Thoracic Society recommends using the lowest effective dose to minimize side effects 6
  • Metered dose inhalers cause fewer systemic effects than nebulizers or oral administration 3

Tachycardia from Albuterol Nebulizers

Mechanism and Frequency

  • Tremor, anxiety, heart pounding, and tachycardia are common dose-dependent side effects that most patients tolerate well, though some individuals are highly sensitive 8
  • Beta-blockers may diminish albuterol effectiveness but are not contraindicated 8

Clinical Monitoring Recommendations

  • Continuous ECG monitoring should be employed during high-dose or continuous nebulization, with a defibrillator immediately available 9
  • Adjust dosing if symptomatic tachycardia develops 9
