Orthotopic Neobladder vs Ileal Conduit: Selection and Comparison

Direct Recommendation

  • The European Association of Urology recommends choosing orthotopic neobladder for younger, motivated patients without urethral tumor involvement who desire to avoid a stoma and can commit to neobladder training; select ileal conduit for patients with urethral/bladder neck involvement, poor manual dexterity, cognitive impairment, or those preferring simpler management 1

Absolute Contraindications to Neobladder

  • The European Urology guidelines state that orthotopic neobladder should not be offered to patients with invasive tumor in the urethra or at the level of urethral dissection 1, 2
  • The National Comprehensive Cancer Network recommends that patients with positive urethral margin on frozen section should not undergo orthotopic neobladder construction 3, 4
  • The National Comprehensive Cancer Network also recommends that carcinoma in situ (CIS) in prostatic ducts is a relative contraindication requiring careful consideration for orthotopic neobladder construction 3, 4

Patient Selection Algorithm

  • The European Urology guidelines recommend selecting neobladder if there is no urethral or bladder neck tumor involvement (confirmed by frozen section) 1, 5
  • The European Urology guidelines also recommend selecting ileal conduit if there is urethral tumor involvement or positive margins 1, 2

Critical Counseling Points

  • The European Urology guidelines state that the final decision between neobladder and ileal conduit should be based on a balanced discussion between patient and surgeon 1
