Diagnostic Approach for Elevated Beta-2 Microglobulin with Normal LDH and Normal CD19

Introduction to Hematologic Malignancies

  • The European Society for Medical Oncology recommends that elevated B2M is incorporated into the FLIPI-2 and PRIMA-PI prognostic scoring systems for follicular lymphoma, where it serves as an independent risk factor regardless of LDH status 1, 2
  • The International Prognostic Scoring System for Waldenström's Macroglobulinemia (IPSSWM) uses B2M as a prognostic marker for survival 3, 4
  • The International Staging System (ISS) for multiple myeloma defines stage III disease as B2M ≥5.5 mg/L 5, 6
  • The National Comprehensive Cancer Network recommends that plasma cells in multiple myeloma are typically CD19-negative, making this diagnosis more consistent with the normal CD19 finding 7

Non-Malignant Causes of Elevated B2M

  • The American Society of Nephrology states that B2M is filtered by the glomerulus and elevated levels commonly reflect decreased renal function 5
  • The European Renal Association recommends checking serum creatinine, BUN, and creatinine clearance to assess renal function 3, 4
  • The American College of Rheumatology suggests that autoimmune diseases, chronic infections, and inflammatory states can elevate B2M 2, 3
  • The National Comprehensive Cancer Network recommends a complete blood count with differential to assess for cytopenias, lymphocytosis, or abnormal cells 2
  • The European Society for Medical Oncology suggests a comprehensive metabolic panel including creatinine and albumin to evaluate renal function and calculate ISS staging if myeloma suspected 5, 6
  • The International Myeloma Society recommends serum protein electrophoresis (SPEP) and immunofixation to detect monoclonal proteins 3, 7
  • The American Society of Clinical Oncology recommends quantitative immunoglobulins (IgG, IgA, IgM) 3, 4
  • The National Comprehensive Cancer Network suggests a 24-hour urine for total protein, UPEP, and urine immunofixation 7

Imaging Studies and Tissue Sampling

  • The European Society for Medical Oncology recommends a CT scan of neck, chest, abdomen, and pelvis to evaluate for lymphadenopathy, organomegaly, or lytic bone lesions 2, 9
  • The National Comprehensive Cancer Network suggests a PET-CT may be useful for lymphoma staging if CT shows suspicious findings 1, 2
  • The International Myeloma Society recommends a skeletal survey if multiple myeloma is suspected 7
  • The American Society of Clinical Oncology recommends bone marrow aspirate and biopsy with flow cytometry, immunohistochemistry, cytogenetics, and FISH testing 2, 5, 7
