Finerenone Initiation, Dosing, and Monitoring in Adults with Type 2 Diabetes and Chronic Kidney Disease

Initiation Criteria

  • Finerenone may be started in adults with type 2 diabetes, CKD (eGFR ≥ 25 mL/min/1.73 m²), and albuminuria (ACR ≥ 30 mg/g) who are already receiving a maximally tolerated ACE‑inhibitor or ARB; serum potassium must be ≤ 4.8 mmol/L (trial criteria) or ≤ 5.0 mmol/L (FDA label) before the first dose【1】.

  • The drug is indicated only as an add‑on to an ACE‑inhibitor or ARB, not as a replacement for these agents【1】.

  • Finerenone does not have glucose‑lowering activity, distinguishing it from SGLT‑2 inhibitors and GLP‑1 receptor agonists【3】.

Dosing Algorithm

Baseline eGFR (mL/min/1.73 m²) Recommended Starting Dose
> 60 20 mg once daily
25 – 60 10 mg once daily
< 25* Initiation not permitted; patients already on therapy may continue if potassium is acceptable*

*Patients with eGFR < 25 mL/min/1.73 m² may remain on finerenone only if they were already receiving it and serum potassium remains within target【1】.

Dose Titration & Potassium Management

  • Potassium check is required 4 weeks after any dose change and at regular intervals thereafter【1】.

  • If serum potassium < 4.8 mmol/L – the dose may be uptitrated from 10 mg to 20 mg daily【1】.

  • If serum potassium ≤ 5.5 mmol/L – maintain the current dose【1】.

  • If serum potassium > 5.5 mmol/L – withhold finerenone; once potassium falls to ≤ 5.0 mmol/L, restart at 10 mg daily【1】.

Monitoring Protocol

  • Baseline: verify serum potassium ≤ 4.8–5.0 mmol/L and record eGFR and ACR ≥ 30 mg/g【1】.

  • 4 weeks after initiation and 4 weeks after any dose adjustment: mandatory serum potassium measurement【1】.

  • Ongoing: continue regular potassium monitoring throughout therapy【1】.

  • eGFR: monitor periodically; finerenone may be continued even if eGFR declines below 25 mL/min/1.73 m², provided potassium remains within target【1】.

  • Albuminuria: baseline ACR ≥ 30 mg/g is required for indication; periodic ACR assessment is advised to gauge treatment response【1】.

Clinical Considerations & Pitfalls

  • Finerenone must not be used as monotherapy; background ACE‑inhibitor or ARB therapy is mandatory【1】.

  • Do not combine finerenone with other mineralocorticoid receptor antagonists (e.g., spironolactone, eplerenone)【1】.

  • Do not delay the 4‑week potassium check after dose changes; this monitoring step is compulsory【1】.

  • Do not automatically discontinue finerenone if eGFR falls below 25 mL/min/1.73 m² during treatment; continuation is acceptable when potassium is controlled【1】.
