Ondansetron Dosing Guidelines for Pediatric Patients

Standard Weight‑Based Dosing

  • For children weighing any amount, administer ondansetron at 0.15 mg per kilogram of body weight per dose, with a maximum single dose of 16 mg for both intravenous (IV) and intramuscular (IM) routes. This recommendation applies to patients ≥ 6 months old. 1

Age Eligibility

  • Ondansetron is recommended for use in children who are at least 6 months of age; younger infants are not covered by current pediatric guidelines. 1

Route‑Specific Recommendations

Intravenous (IV) Administration

  • IV is the preferred route for severe presentations (e.g., severe food‑protein‑induced enterocolitis syndrome with hypotension or extreme lethargy). The dose remains 0.15 mg/kg with the 16 mg ceiling. 1

Intramuscular (IM) Administration

  • IM may be used when IV access is unavailable or delayed, delivering the same 0.15 mg/kg dose (max 16 mg). It is suitable for moderate symptoms requiring rapid treatment without IV access. 1

Maximum Daily Dosing in Non‑Hepatic Impairment

  • In patients without hepatic impairment, the 0.15 mg/kg dose can be repeated as clinically needed; acute‑care guidelines do not specify an absolute daily maximum. 1

Clinical Context: Food Protein‑Induced Enterocolitis Syndrome (FPIES)

Symptom Severity Recommended Route Dose (per kg) Age Requirement
Mild (1–2 episodes of vomiting, no lethargy) IM 0.15 mg/kg (max 16 mg) ≥ 6 months
Moderate (>3 episodes of vomiting, mild lethargy) IM 0.15 mg/kg (max 16 mg) ≥ 6 months
Severe (>3 episodes of vomiting, severe lethargy, hypotension) IV (or IM if IV delayed) 0.15 mg/kg (max 16 mg) ≥ 6 months

*These dosing strategies are based on the same weight‑based recommendation and are tailored to symptom severity in FPIES. 1

Ondansetron Dosing Guidelines for Children

Dosing Recommendations

  • For a 20kg child, the recommended dose of ondansetron is 0.15 mg/kg, which equals 3mg per dose, with a maximum single dose of 16mg 2, 3
  • The American Academy of Allergy, Asthma, and Immunology recommends 0.15 mg/kg/dose with a maximum of 16 mg per dose for children ≥6 months via intravenous (IV) administration 2
  • For intramuscular (IM) administration, the same dosing as IV is recommended: 0.15 mg/kg/dose with a maximum of 16 mg 3
  • For a 20kg child, the IM dose equals 3mg 3

Clinical Applications

  • For mild symptoms of Food Protein-Induced Enterocolitis Syndrome (FPIES), consider ondansetron 0.15 mg/kg IM if the child is ≥6 months old 2
  • For moderate symptoms of FPIES, administer ondansetron 0.15 mg/kg IM if the child is ≥6 months old 3
  • For severe symptoms of FPIES, administer ondansetron 0.15 mg/kg IV if the child is ≥6 months old; if IV access is difficult, use the IM route 2

Important Considerations

  • Special caution is warranted in children with heart disease due to potential QT interval prolongation when administering ondansetron 2
