Pleural Drainage Systems

Mechanism of Action

  • The Pleurevac system operates through a three-chamber mechanism, including a collection chamber, water seal chamber, and suction control chamber, to evacuate air and fluid from the pleural space, allowing lung re-expansion 1

Indications and Guidelines

  • The American Thoracic Society recommends using small-caliber tubes (14F or smaller) to reduce complications in patients with symptomatic pleural effusions 1, 7
  • The guideline suggests avoiding removal of more than 1.5L of fluid at one time to prevent re-expansion pulmonary edema 1, 7

Management and Complications

  • The use of ultrasound guidance for all pleural interventions is recommended to improve success rates and reduce complications, with a significant reduction in pneumothorax rates (1.0% vs 8.9% without guidance) 1, 7
  • The risk of re-expansion pulmonary edema is a potential complication if more than 1.5L of fluid is drained rapidly, highlighting the need for controlled drainage 1, 7

Special Considerations

  • Contraindications for pleural drainage include a trapped lung without possibility of re-expansion, active pleural infection with multiple loculations, and patients who cannot tolerate the procedure due to hemodynamic instability 1, 6, 8
  • Potential complications of pleural drainage include pneumothorax, especially without ultrasound guidance, and infection or empyema, with reported rates of 2.2-2.8% in series with permanent catheters 6, 7, 8
