Management of Fanconi Syndrome

Supportive Care

  • The American Academy of Pediatrics recommends fluid and electrolyte replacement with intravenous 10% dextrose/normal saline solutions at 1.5-2.0 times maintenance rate to maintain normoglycemia in patients with Fanconi syndrome 1
  • The National Institute of Diabetes and Digestive and Kidney Diseases suggests initiating cysteamine therapy as soon as diagnosis is confirmed, with a starting dose of 1 mg/kg/day, which may increase to 2 mg/kg if no improvement is seen in patients with cystinosis-related Fanconi syndrome 1

Disease-Specific Management

  • The Cystinosis Research Foundation recommends nutritional management, including restriction of phenylalanine and tyrosine intake, use of medical foods devoid of phenylalanine and tyrosine, and ensuring adequate protein intake (higher than normal requirements) in patients with cystinosis-related Fanconi syndrome 1
  • The Fanconi Anemia Research Fund suggests hematologic monitoring, including complete blood count at diagnosis and frequent monitoring, and bone marrow aspirate and biopsy at diagnosis and annually thereafter in patients with Fanconi anemia-related manifestations 2
  • The National Cancer Institute recommends cancer surveillance, including monthly oral self-examinations, biannual dental examinations, and annual head and neck squamous cell carcinoma evaluation by otolaryngologist beginning in early adolescence in patients with Fanconi anemia-related manifestations 2, 3, 4
  • The American College of Obstetricians and Gynecologists suggests annual gynecologic examination starting in adolescence in patients with Fanconi anemia-related manifestations 3, 4
  • The Centers for Disease Control and Prevention recommends HPV vaccination per standard schedule in patients with Fanconi anemia-related manifestations 3
  • The American Society of Hematology suggests treatment options for bone marrow failure, including androgen therapy, lower-dose hematopoietic stem cell transplantation regimens, and avoidance of DNA-damaging agents in patients with Fanconi anemia-related manifestations 2, 3, 4, 5

Treatment of Fanconi Syndrome

Underlying Cause Management

  • In tyrosinemia type I-associated Fanconi syndrome, NTBC (nitisinone) treatment with dietary restriction of phenylalanine/tyrosine is associated with reversal or prevention of tubulopathy in nearly all cases within weeks 6

Monitoring and Follow-up

  • Regular assessment of renal function through blood and urine tests, including electrolytes, glucose, amino acids, and phosphate, is recommended for patients with Fanconi syndrome, as per the guidelines from the Genetics in Medicine society 6
  • Renal ultrasound to monitor kidney structure, and bone X-rays and densitometry to assess for rachitic changes from hypophosphatemia, are also recommended for patients with Fanconi syndrome 6

Special Considerations

  • For tyrosinemia type I patients who have undergone liver transplantation, periodic screening for renal disease is recommended as SA (succinylacetone) excretion continues post-transplant, according to the Genetics in Medicine society 6
