Pazopanib in Advanced Clear Cell Renal Cell Carcinoma

Introduction to Pazopanib

  • The National Comprehensive Cancer Network recommends pazopanib 800 mg orally daily as a standard first-line treatment option for patients with advanced clear cell RCC, based on a pivotal phase III trial that demonstrated significant improvement in progression-free survival (PFS) compared to placebo 1, 2

Efficacy and Safety

  • In a landmark trial, pazopanib significantly improved PFS compared to placebo in patients with advanced clear cell RCC who had received 0-1 prior treatments, including both treatment-naïve patients and those previously treated with cytokines 1, 2, 3, 4
  • The trial established pazopanib as a category 1 recommendation after cytokine therapy, with a median PFS of 7.4 months versus 4.2 months with placebo in cytokine-pretreated patients 3, 4
  • No significant difference in overall survival was observed between pazopanib and placebo groups, representing an important limitation of the trial findings 1, 2

Mechanism of Action and Toxicities

  • Pazopanib is an oral multitargeted tyrosine kinase inhibitor that works primarily through antiangiogenic properties by inhibiting vascular endothelial growth factor receptors (VEGFR-1, -2, and -3), platelet-derived growth factor receptor-α and -β (PDGFR-α and -β), and stem cell factor receptor (c-KIT) 1, 2
  • Hepatotoxicity is the most notable severe toxicity, with elevated alanine transaminase (ALT) in 30% and aspartate transaminase (AST) in 21% of patients, requiring liver function monitoring before treatment initiation, regular monitoring during treatment, and dose interruption, reduction, or discontinuation based on severity 1, 2, 5

Treatment Recommendations

  • The National Comprehensive Cancer Network guidelines list pazopanib as a category 2A, other recommended regimen for patients with clear cell RCC across all risk groups (favorable, intermediate, and poor risk) 1, 2
  • Pazopanib is a category 1 recommendation after cytokine therapy, and a category 3 recommendation after tyrosine kinase inhibitor failure (no data available in this setting) 3, 4

Comparative Efficacy

  • The COMPARZ noninferiority study demonstrated similar safety and efficacy between pazopanib and sunitinib, with pazopanib showing superior health-related quality of life compared to sunitinib 1, 2
