Methylphenidate Extended-Release Formulations

Dosing Characteristics

  • The American Academy of Child and Adolescent Psychiatry recommends Jornay PM, which is administered in the evening, unlike other methylphenidate extended-release formulations that are typically taken in the morning 1, 2
  • Jornay PM's evening dosing allows for medication to be active upon awakening, addressing the common challenge of morning symptom control 2, 3
  • Traditional extended-release methylphenidate formulations, like Concerta, Ritalin LA, and Metadate CD, are dosed in the morning and may not provide adequate coverage for early morning symptoms 1, 2
  • Standard extended-release methylphenidate formulations typically provide 8-12 hours of coverage, whereas Jornay PM's delayed-release technology provides coverage beginning upon awakening and lasting throughout the day 2, 3

Pharmacokinetic Differences

  • Jornay PM utilizes a delayed-onset technology that delays the initial release of methylphenidate until the early morning hours, approximately 10-12 hours after evening administration 2
  • Traditional extended-release methylphenidate formulations like Concerta provide an ascending plasma concentration pattern but begin working within 30-90 minutes of morning administration 2
  • Older sustained-release formulations, like Ritalin-SR, have been found to be less effective than immediate-release versions due to delayed onset and lower peak plasma concentrations 1, 2

Side Effect Profiles

  • All methylphenidate formulations share common side effects, including insomnia, decreased appetite, headache, and potential cardiovascular effects, according to the National Comprehensive Cancer Network 4, 5
  • Agitation and insomnia are the most commonly reported side effects across all methylphenidate formulations 4, 5
  • Rare but serious side effects for all formulations include hypertension, palpitations, arrhythmias, confusion, psychosis, tremor, and headache 5
  • Evening dosing of Jornay PM may reduce the risk of insomnia compared to other extended-release formulations taken in the morning 2

Clinical Considerations for Selection

  • Jornay PM may be particularly beneficial for patients who struggle with early morning functioning, including getting ready for school or work 2, 3
  • The selection between different formulations should consider the patient's daily schedule, symptom pattern throughout the day, and previous response to methylphenidate products 1, 2
  • Compliance challenges with traditional formulations may be addressed by using either Jornay PM or long-acting morning-dosed formulations 1, 2

Potential Pitfalls and Caveats

  • Methylphenidate in any formulation should be avoided in patients with uncontrolled hypertension, underlying coronary artery disease, and tachyarrhythmias 5
  • The unique evening dosing of Jornay PM requires careful patient education to ensure proper administration timing for optimal effectiveness 2
