Evidence‑Based Omeprazole Dosing Recommendations

Helicobacter pylori Eradication

  • In adults, a 10‑day triple‑therapy regimen consisting of omeprazole 20 mg twice daily, amoxicillin 1000 mg twice daily, and clarithromycin 500 mg twice daily achieves H. pylori eradication; this regimen is supported by a Gastroenterology study (2021) 1.
  • In adults, a 14‑day dual‑therapy regimen of omeprazole 40 mg once daily combined with clarithromycin 500 mg three times daily is an alternative eradication strategy, also reported in Gastroenterology (2021) 1.

Optimal Timing of Omeprazole Administration

  • For twice‑daily dosing, taking each omeprazole tablet 30 minutes before the morning and evening meals maximizes acid suppression, as demonstrated in the same Gastroenterology (2021) investigation 1.

Eosinophilic Esophagitis (EoE)

  • In adult patients with EoE, omeprazole 20 mg twice daily for a minimum of 8–12 weeks is recommended before histologic response assessment; this recommendation is based on two Gut (2022) studies [2][3].
  • In pediatric patients with EoE, a dose of 1 mg/kg twice daily (maximum 40 mg twice daily) has been shown to be effective, according to two Gut (2022) publications [4][5].

Long‑Term Safety in Infants

  • Prolonged omeprazole use in infants is associated with an increased risk of lower respiratory tract infections, as reported in a Pediatrics cohort study (2013) 6.
