Timing of Pulmonary Function Tests After Thoracic Surgery for Pneumothorax

Contraindications and Precautions

  • The British Thoracic Society recommends that patients should not undergo pulmonary function tests during the acute phase of pneumothorax or immediately after thoracic surgery, as forced expiratory maneuvers can produce inaccurate results and potentially worsen the patient's condition 1, 2.
  • Patients should avoid spirometry for at least 2 weeks after resolution of pneumothorax, according to guidelines 3.

Post-Surgery Waiting Period

  • The British Thoracic Society guidelines establish that patients should not travel by air within 6 weeks after thoracic surgery or resolution of spontaneous pneumothorax, to allow for adequate pleural healing and stabilization of post-surgical lung function 4, 5, 6.
  • A 6-week waiting period is recommended before considering pulmonary function tests or activities involving significant pressure changes, to ensure adequate pleural healing and stabilization of post-surgical lung function 4.

Radiological Follow-Up

  • A follow-up chest radiograph should be performed 2-4 weeks after surgery to confirm complete resolution of pneumothorax and adequate lung expansion, before considering pulmonary function tests 7, 3, 8.
  • After radiological confirmation of resolution, patients should wait at least an additional 7 days to exclude early recurrence 3.

Clinical Utility of Pulmonary Function Tests

  • Pre-operative pulmonary function tests are useful for patient selection and decision-making regarding prevention of recurrences, but not for acute management, according to the American College of Chest Physicians 1, 2.
  • In patients with relatively good lung function, pre-operative pulmonary function tests help stratify the risk of tolerance to future episodes 1, 2.
  • Conversely, patients with poor lung function should be advised about the high risk of complications with recurrences 1, 2.

Practical Algorithm

Important Precautions

  • Patients should avoid heavy lifting for at least 2 weeks after resolution of pneumothorax 3.
  • BiPAP should be avoided while any residual pneumothorax exists, as it may worsen the condition 3.
  • In patients with cystic fibrosis, management is more aggressive and follow-up should be closer 7, 3, 8.
