Amorolfine Nail Lacquer Treatment for Onychomycosis

Application and Efficacy

  • The British Journal of Dermatology recommends applying amorolfine 5% nail lacquer once weekly for 6-12 months after filing down diseased nail areas, achieving approximately 50% cure rates in distal fingernail and toenail onychomycosis 1
  • Filing the nail first by removing as much diseased nail as possible before each application is recommended 1, 2
  • Applying once weekly is equally effective as twice weekly 1, 2
  • The treatment should continue for 6-12 months depending on severity and response 1, 2, 3
  • The medication remains in the nail for 14 days after each application 1

Indications and Contraindications

  • Amorolfine should be reserved for specific limited presentations, such as superficial white onychomycosis (SWO) and early distal lateral subungual onychomycosis (DLSO) with less than 80% nail plate involvement and no lunula involvement 2, 3
  • Amorolfine can be used when systemic antifungals are contraindicated due to drug interactions, liver disease, or patient preference 2, 3

Expected Outcomes

  • Clinical improvement does not equal mycological cure, with cure rates typically 30% lower than apparent clinical improvement 1, 2
  • Clinical cure rates are approximately 50% 1, 2

Side Effects and Comparative Context

  • Adverse effects are rare and limited to local burning, pruritus, and erythema 1, 2
  • Amorolfine is effective for limited disease, but its place in the treatment hierarchy should be understood, with ciclopirox 8% lacquer having lower cure rates and efinaconazole 10% achieving similar mycological cure rates 1, 2, 4
  • Oral terbinafine remains the gold standard for extensive disease with higher efficacy 5
