Calcified Lymph Nodes in the Mediastinum

Clinical Significance and Management

  • The American College of Radiology recommends that mediastinal lymph nodes >10mm in short axis are considered abnormal 3, 4
  • The American College of Radiology suggests that for asymptomatic calcified nodes <15mm, no further follow-up is needed 3, 4
  • For symptomatic patients with calcified lymph nodes, bronchoscopy is the definitive diagnostic test, according to the American College of Chest Physicians 2
  • The Infectious Diseases Society of America recommends that antifungal treatment is not indicated for asymptomatic calcified nodes (histoplasmomas) 1
  • The American College of Radiology notes that in young male patients with enlarged mediastinal nodes, consideration should be given to lymphoma or metastatic germ cell tumors even if calcification is present 3, 4

Radiographic Features and Evaluation

  • The American College of Radiology states that nodes with central low attenuation and peripheral rim enhancement suggest active disease, while homogeneous and calcified nodes suggest inactive disease 3, 4
  • The American College of Chest Physicians recommends that when broncholithiasis is suspected but not confirmed, bronchoscopy is essential for definitive diagnosis 2

Management Algorithm

  • The Infectious Diseases Society of America recommends that for asymptomatic calcified mediastinal lymph nodes, no treatment or follow-up is required 1
  • The American College of Chest Physicians suggests that for symptomatic calcified mediastinal lymph nodes, if broncholithiasis is suspected, bronchoscopy should be performed for diagnosis and potential removal 1, 2
  • The Infectious Diseases Society of America recommends that for mediastinal fibrosis, consideration should be given to intravascular stents for vessel obstruction, and itraconazole may be considered if distinction between mediastinal granuloma and fibrosis is unclear 1
