Cyproheptadine Dosing for Pediatric Oncology Patients

Clinical Evidence and Administration

  • The American Academy of Pediatrics recommends administering cyproheptadine orally as tablets, or crushed and given via nasogastric tube if needed, with response evaluation after 4 weeks of treatment 1, 2
  • For serotonin syndrome treatment, pediatric dosing is 0.25 mg/kg per day, whereas for appetite stimulation in cancer patients, dosing follows a specific schedule with divided doses throughout the day 1, 2

Alternative Therapies

  • The National Comprehensive Cancer Network suggests considering megestrol acetate as second-line therapy for pediatric patients who do not respond to cyproheptadine after 4 weeks, despite its higher risks including potential cortisol suppression and hyperlipidemia 3, 4
